Endoscopic Keyhole Surgery for Bertolotti’s Syndrome:

In some cases, when conservative treatments like physical therapy, medications, or injections do not alleviate the symptoms, surgery may be considered. An endoscopic keyhole surgery approach may be used as a minimally invasive option.

Here’s how the procedure typically works:

  1. Small Incision: A single small “keyhole” incision of less than 1 cm is planned under xray guidance. The working channel is inserted using a minimally invasive fashion , ensuring minimal tissue disruption and trauma while accessing the target area.
  2. Endoscope and Instruments: An endoscope, a small tube equipped with a 4K camera, is inserted through the working channel to provide real-time visualisation of the affected area on a high-definition monitor. Specialised precision instruments like drill are passed through the endoscope and the procedure is carried out under direct vision in high magnification.
  3. Decompression: The goal of the surgery is to excise the offending pseudo joint in a minimally invasive approach to alleviate the pain. This is done with the use of specialised burr, whereby the cartilage is completely removed under vision. The abnormal joint is resected to prevent any further irritation leading to disabling pain.
  4. Recovery: Being a minimally invasive surgery, the recovery and post operative rehabilitation are faster and has lesser post-operative pain compared to traditional open surgery. The small Keyhole incision also reduces the risk of infection and scarring.

Risks:

  • Injury to Nerves or Surrounding Structures: As with any surgery, there’s a risk of nerve damage, bleeding, or infection.
  • Recurrence of Symptoms: In some cases, the condition may return if the abnormal bony connection is not fully addressed.

The procedure is often recommended when conservative treatments like physical therapy, medications, or injections haven’t provided relief, and the disc herniation is causing significant pain, numbness, or weakness.
